Human Factors
- Has our operational environment been assessed for Human Factors influencers?
- Do we understand what the true Human Error Probabilities are for each task or executive action event?
- Have we identified the real Performance Shaping Factors (PSF's) within our operational profile at site, and how they are affecting our performance?
Functional Safety Engineering
- Have we correctly assessed (and continue to assess), the hazards and consequences associated with our plant equipment/site machinery, to meet the requirements of our machine or process safety regulatory commitments and/or performance targets? (IEC 61511/61508, ISO 13849, EU Machinery Directive, etc.)
- Are we meeting our Functional Safety Management commitments with our procedures and management systems?
- Are we correctly and confidently executing Proof testing at the required frequencies, to validate and have confidence in our safety systems?
Cybersecurity
- Have we documented and accurate Risk Assessments carried out to meet our regulatory commitment, and our security level targets? (IEC 62443, OG-0086, etc.)
- Do we perform accurate and competent Security Level verification and validation at site?
- Can we say with confidence we have assigned appropriate Countermeasures for our active Operational Threats, with both remote and physical security in mind?
- Do we have an active Security Case for our site, documenting current performance against our Cyber Operational Integrity (COI) target metrics?
Site Operational Maintenance
- Does the site (or organisation), have a defined and active Maintenance strategy?
- How have we defined our metrics for maintenance against operational performance? (Overall Equipment Efficiency, Reactive Maintenance, Condition Based Monitoring, Preventative Maintenance, etc.)
- Do our leading and lagging indicators inform our maintenance delivery, ultimately staying ahead of the performance curve, and how are we tracking these metrics?
Training & Competency
- Do we have an operational competency matrix or competency management system, that accurately represents, the roles and responsibilities of our operational personnel and teams, along with being representative of the needs for the site?
- Do we track and assess competency at site, and more importantly, maintain competency at site?
- Have we defined what competency in each technically responsible area looks like, and have we assessed/certified such competency for accurate development of operational teams?
